Proven Peer reviewed Evidence Based Impact:
Preferred Population Health Management (PPHM) brings to market an evidence based brain care bundle developed, tested, and implemented by Indiana University. Currently two health care systems, Eskenazi health and Indiana University Health, have used this brain care bundle to serve more than 4000 patients. Our evidence based, peer reviewed brain care bundle has accomplished the following impact on health outcomes, care quality and cost.

45% reduction in Emergency department visits:

  • Only 28% of patients served by our brain care bundle visited the emergency department compared to 50% of patients in the usual primary care setting.

28% reduction in length of hospital stay:

  • Patients served by our brain care bundle spent two fewer days in the hospital - a reduction from seven to five days on average.

45% fewer 30 day readmissions:

  • Only 11% of patients served by our brain care bundle were readmitted to the hospital within 30 days, compared to 20% in the usual primary care setting - a key quality indicator.

52% fewer inappropriate (and potentially harmful) medications:

  • Only 19% of patients served by our brain care bundle received inappropriate and potentially harmful medications, compared to 40% in the usual primary care setting - a 52% reduction.

Better control of diabetes:

  • Diabetic patients served by our brain care bundle had better blood sugar control, with 78% scoring under 8 on their A1C blood glucose testing, compared to 51% of patients in the usual primary care setting.

Better control of  high cholesterol:

  • Almost twice as many patients with hyperlipidemia served by our brain care bundle succeeded in lowering their LDL to below 130 (45% compared to 23%).

Lower caregiver burden:

  • 90% of caregivers (loved ones) served by our brain care bundle had no stress within 12 months.
